This information should always be passed from doctor to doctor and in most cases the doctors will ask you to sign off on this transfer of information. One of the problems with medical-records is that they tend to be paper. This can be tiresome to update and often does not follow the patient well enough. There are some strides being made to help this though. For example, much of the medical-records are now being put on computers in databases that doctors share. Also, you will find that many of the doctors are using electronic means for recognizing patients and conditions.
